Application form is available from the Office of the institute or you can directly download from the website itself. Filled up application should be submitted to The Director/ The Principal, Madathil Ayurveda Hospital and Nursing Institute, Piravom, Ernakulam, Kerala-686664.Attach one passport size photo and copies of certificates of passed examinations.
Admission
The candidates selected shall report to the office in person, with guardian on the date & time prescribed. On the time of admission the candidates have to submit their
1) S. S. L. C book Orginal (or equivalent ) which shall be returned only after the completion of the course.
2) 6 Passport size photo.
3) Copy of ration card(For fees concession of BPL candidates) Fees can be paid as 3 installments. 1st Installment should pay at the time of admission in office or through bank transfer.
Discipline
The students are expected to behave properly to the Patients, Teachers, Doctors, Therapist and Staffs. They should be calm and quiet as the Classes were conducting in the hospital itself. They should take care of all the equipments, instruments, furniture and accessories which are handlef by them. If any damage or loss occurs, the student who is responsible for it will be made to reimburse the rate. Misconduct on the part of the students will be expelled from the institution without any further consideration of the institute in general and if any action is taken against such a student, the decision of the Management wil be the ultimate
